These Terms of Service ("Terms") govern your use of the services provided by Synergy Mind & Wellness P.L.L.C. and Aaron Sahle, MSN, APRN, PMHNP-BC, FNP-BC, FNP-C ("Service Providers", "We", "Us"). By accessing or using our services, you agree to comply with these Terms. If you do not agree to these Terms, you may not use our services.

1. Services Provided

Synergy Mind & Wellness P.L.L.C., led by Aaron Sahle, provides mental health and wellness services, including but not limited to psychotherapy, psychiatric evaluations, medication management, and wellness consultations. The services provided are intended for educational and clinical purposes only. You acknowledge that our services are not a substitute for emergency or urgent care, and should you experience a medical emergency, you should seek appropriate medical attention immediately.

2. Appointment Scheduling and Cancellations

Appointments must be scheduled in advance through our online scheduler, by phone at (888) 418-5322, or by email at patientsupport@synergymind.health. You agree to provide accurate and current information when scheduling and to keep all scheduled appointments, or notify us at least 24 hours in advance if you are unable to attend. Cancellations made with less than 24 hours' notice may be subject to a cancellation fee as outlined in our payment policy.

3. Privacy and Confidentiality

We are committed to maintaining your privacy and the confidentiality of your health information. We will not disclose any personal health information without your consent, except as required by law or as needed to provide services. Our privacy practices are governed by applicable laws, including the Health Insurance Portability and Accountability Act (HIPAA). How this website and our messaging program handle information is described in our Privacy Policy.

4. Payment and Insurance

You are responsible for payment for services rendered, including any applicable fees, insurance co-pays, or out-of-pocket costs. Payment is due at the time of service unless prior arrangements are made. We accept various forms of payment, including credit cards and insurance. If you plan to use insurance, it is your responsibility to verify that our services are covered under your insurance plan.

5. Professional Conduct

We strive to provide the highest level of care in a professional and ethical manner. You agree to communicate openly and respectfully with our providers and staff. Disruptive, abusive, or inappropriate behavior may result in termination of services.

6. No Guarantee of Results

While we are committed to providing you with the best possible care, we do not guarantee specific results or outcomes from our services. Your progress and outcomes depend on a variety of factors, including your engagement in the treatment process.

7. Telehealth Services

Telehealth services, including virtual appointments, may be offered as part of your treatment plan. By engaging in telehealth services, you agree to abide by all applicable laws and regulations governing telemedicine and telehealth services in your location. You are responsible for ensuring that your technology and internet connection are adequate for these services.

8. Intellectual Property

The content, materials, and information provided by Synergy Mind & Wellness P.L.L.C., including any written materials, videos, and other educational content, are the intellectual property of Synergy Mind & Wellness P.L.L.C. and are protected by copyright laws. You may not reproduce, distribute, or use any of these materials for commercial purposes without written consent.

9. Limitation of Liability

To the fullest extent permitted by law, Synergy Mind & Wellness P.L.L.C. and Aaron Sahle, MSN, APRN, PMHNP-BC, FNP-BC, FNP-C will not be liable for any direct, indirect, incidental, or consequential damages arising from your use of our services, including any loss of data, business interruption, or medical complications.

10. Termination of Services

Either party may terminate the professional relationship at any time, with or without cause, upon written notice. In the event of termination, any outstanding fees for services rendered must be paid in full.

11. Dispute Resolution

Any disputes arising from these Terms will be resolved through binding arbitration or in accordance with applicable laws. We encourage communication to resolve any concerns before resorting to legal actions.

12. Changes to These Terms

Synergy Mind & Wellness P.L.L.C. and Aaron Sahle reserve the right to modify or update these Terms at any time. All changes will be effective upon posting on our website or notifying you in writing.

13. Governing Law

These Terms are governed by and construed in accordance with the laws of the state in which Synergy Mind & Wellness P.L.L.C. is located, without regard to its conflict of law provisions.

SMS Terms and Conditions

These terms apply to the text messaging program operated by Synergy Mind & Wellness PLLC.

1. SMS for Consent Communication

We will not share your phone number with any third parties for marketing purposes.

2. Types of SMS Communications

If you opt in to receive messages from Synergy Mind & Wellness PLLC, you may receive texts for conversational purposes. These include appointment reminders and confirmations, scheduling, care coordination, prescription and refill updates, billing notices, and practice announcements. Standard message rates may apply.

3. Message Frequency

You may receive approximately 2 to 10 SMS messages per month.

4. Carrier Charges

Message and data rates may apply based on your mobile carrier and location.

5. Opt-In Methods

Additional Messaging Terms

Because we are a healthcare practice, a few things go beyond the standard messaging terms above.

What you are consenting to

By providing your mobile number and opting in, you agree that Synergy Mind & Wellness PLLC may send you text messages (SMS/MMS) related to your care and our services. Marketing messages require separate consent.

Privacy and HIPAA considerations

Text messaging has privacy limitations. We aim to use the minimum necessary information and approved workflows, but SMS is not always fully secure. Do not send highly sensitive information by text.

Not for emergencies

Text messaging is not monitored 24 hours a day and should not be used for urgent or emergency concerns. If you are experiencing a medical emergency, call 911. For mental health crisis support, call or text 988, or text HOME to 741741.

Eligibility

You must be at least 18 years old to opt in yourself. A parent or guardian can enroll someone younger.
