Every evaluation ends with a recommended next step you could repeat back in one sentence. Medication, labs, a referral, or a follow-up date.

Psychiatric evaluation
Photo ID and insurance card, a list of current medications with doses, and any prior records or medication history you can gather. Partial is fine. The hour has room to fill gaps.
Sick visit
A quick symptom timeline helps. When it started, any temperatures you've taken, and what you've already tried. Bring your medication list too, since it changes what's safe to add.
Weight loss consult
Your medication list, recent labs if you have them, and a plain picture of what you've tried before. The evaluation starts from your actual history, not a template.
