Privacy Policy

This policy explains what Synergy Mind & Wellness P.L.L.C. collects through this website and through our text messaging program, how we use it, and who we share it with.

The health information we create and keep while caring for you is a separate matter, governed by HIPAA and by the practice's Notice of Privacy Practices. Ask our office for a copy of that notice.

This website collects very little. There is no contact form, no account login, and no newsletter signup on it.

If you email us or call the number listed on this site, we receive what you choose to send. That usually means your name, your phone number, your email address, and the reason you are reaching out.

If you book an appointment, scheduling runs through Charm, the electronic health record the practice uses. The calendar on our appointments page is embedded from Charm, and what you enter there goes to Charm under our agreement with them and becomes part of your record.

If you give us your mobile number and opt in to text messages, we keep that number and a record of your consent.

We use what you give us to answer you, to schedule and confirm appointments, to coordinate your care, to handle billing and insurance, and to meet our legal and professional obligations.

We do not use it to build advertising profiles, and we do not sell it.

We do not sell personal information.

We share information with the service providers who help us run the practice, such as our electronic health record vendor, our telephone and messaging provider, and our payment processor. Each receives only what it needs to do its job, and each is bound by confidentiality obligations and by applicable law.

SMS opt-in and phone numbers collected for SMS consent will not be shared with any third party or affiliate company.

No mobile information is shared with third parties or affiliates for marketing or promotional purposes.

We disclose information when the law requires it, for example under a court order or a mandatory reporting obligation.

4. Text Messaging

Our text messages are administrative. They cover appointment reminders, scheduling, care coordination, refill and billing notices, and practice announcements. Marketing by text requires separate consent, which we do not currently collect.

You can stop the messages at any time by replying STOP, and you can restart them by replying START. Reply HELP for assistance. Message and data rates may apply. Message frequency may vary.

The health information we create and keep as part of your care is protected health information under HIPAA. This website policy does not replace the practice's Notice of Privacy Practices, which describes your rights over your record and the ways that record may be used and disclosed. Ask our office for a copy.

Text messaging has real privacy limits. Messages travel over ordinary carrier networks, so we keep them short and administrative. Do not send sensitive health details by text.

This site runs no advertising trackers, no analytics scripts, and no social media pixels. We do not use cookies to follow you across other websites.

The only thing the site stores in your browser is a single session entry that remembers you closed the help prompt in the corner of the page. It holds nothing about you and it clears when you close the browser.

Loading our appointments page contacts Charm's servers, because the calendar is embedded from them. What happens inside that calendar is covered by Charm's own terms.

We keep administrative and technical safeguards suited to the information we hold, and this site is served over an encrypted connection. No method of transmitting information over the internet is completely secure, so we do not claim absolute security.

You can leave the text messaging program at any time by replying STOP to any message.

You can ask what information we hold about you, ask us to correct it, or ask us to delete information we are not required to keep. Requests that concern your medical record follow the process described in the Notice of Privacy Practices. Use the contact details below to start any of these.

This website is not directed to children under 13, and we do not knowingly collect their information through it. Care for patients who are minors is arranged through a parent or guardian.

We may update this policy. The current version is always posted here with its effective date at the top, and a material change will be noted on this page.
