Terms of Service

These Terms of Service ("Terms") govern your use of the services provided by Synergy Mind & Wellness P.L.L.C. and Aaron Sahle, MSN, APRN, PMHNP-BC, FNP-BC, FNP-C ("Service Providers", "We", "Us"). By accessing or using our services, you agree to comply with these Terms. If you do not agree to these Terms, you may not use our services.

1. Services Provided

Synergy Mind & Wellness P.L.L.C., led by Aaron Sahle, provides mental health and wellness services, including but not limited to psychotherapy, psychiatric evaluations, medication management, and wellness consultations. The services provided are intended for educational and clinical purposes only. You acknowledge that our services are not a substitute for emergency or urgent care, and should you experience a medical emergency, you should seek appropriate medical attention immediately.

2. Appointment Scheduling and Cancellations

Appointments must be scheduled in advance via [methods of scheduling]. You agree to provide accurate and current information when scheduling and to keep all scheduled appointments, or notify us at least 24 hours in advance if you are unable to attend. Cancellations made with less than 24 hours' notice may be subject to a cancellation fee as outlined in our payment policy.

3. Privacy and Confidentiality

We are committed to maintaining your privacy and the confidentiality of your health information. We will not disclose any personal health information without your consent, except as required by law or as needed to provide services. Our privacy practices are governed by applicable laws, including the Health Insurance Portability and Accountability Act (HIPAA).

4. Payment and Insurance

You are responsible for payment for services rendered, including any applicable fees, insurance co-pays, or out-of-pocket costs. Payment is due at the time of service unless prior arrangements are made. We accept various forms of payment, including credit cards and insurance. If you plan to use insurance, it is your responsibility to verify that our services are covered under your insurance plan.

5. Professional Conduct

We strive to provide the highest level of care in a professional and ethical manner. You agree to communicate openly and respectfully with our providers and staff. Disruptive, abusive, or inappropriate behavior may result in termination of services.

6. No Guarantee of Results

While we are committed to providing you with the best possible care, we do not guarantee specific results or outcomes from our services. Your progress and outcomes depend on a variety of factors, including your engagement in the treatment process.

7. Telehealth Services

Telehealth services, including virtual appointments, may be offered as part of your treatment plan. By engaging in telehealth services, you agree to abide by all applicable laws and regulations governing telemedicine and telehealth services in your location. You are responsible for ensuring that your technology and internet connection are adequate for these services.

8. Intellectual Property

The content, materials, and information provided by Synergy Mind & Wellness P.L.L.C., including any written materials, videos, and other educational content, are the intellectual property of Synergy Mind & Wellness P.L.L.C. and are protected by copyright laws. You may not reproduce, distribute, or use any of these materials for commercial purposes without written consent.

9. Limitation of Liability

To the fullest extent permitted by law, Synergy Mind & Wellness P.L.L.C. and Aaron Sahle, MSN, APRN, PMHNP-BC, FNP-BC, FNP-C will not be liable for any direct, indirect, incidental, or consequential damages arising from your use of our services, including any loss of data, business interruption, or medical complications.

10. Termination of Services

Either party may terminate the professional relationship at any time, with or without cause, upon written notice. In the event of termination, any outstanding fees for services rendered must be paid in full.

11. Dispute Resolution

Any disputes arising from these Terms will be resolved through binding arbitration or in accordance with applicable laws. We encourage communication to resolve any concerns before resorting to legal actions.

12. Changes to These Terms

Synergy Mind & Wellness P.L.L.C. and Aaron Sahle reserve the right to modify or update these Terms at any time. All changes will be effective upon posting on our website or notifying you in writing.

13. Governing Law

These Terms are governed by and construed in accordance with the laws of the state in which Synergy Mind & Wellness P.L.L.C. is located, without regard to its conflict of law provisions.

SMS/Text Messaging Consent Policy

1. What you are consenting to

By providing your mobile number and opting in (for example, by checking a consent box, signing a consent form, or confirming by text), you agree that Synergy Mind & Wellness PLLC may send you text messages (SMS/MMS) related to your care and our services. These messages may include appointment reminders, care coordination, prescription updates, billing notices, and practice announcements. Marketing messages require separate consent.

2. Message frequency

Message frequency may vary based on your needs and activity (appointments, follow-ups, refills, and administrative items).

3. Opt-out instructions (STOP)

You can opt out at any time by replying STOP to any message. After you opt out, you will no longer receive SMS messages from us unless you re-enroll.

5. Message and data rates

Message and data rates may apply based on your mobile plan and carrier.

6. Privacy and HIPAA considerations

Text messaging has privacy limitations. We aim to use the minimum necessary information and approved workflows, but SMS is not always fully secure. Do not send highly sensitive information by text.

7. Not for emergencies

Text messaging is not monitored 24/7 and should not be used for urgent or emergency concerns. If you are experiencing a medical emergency, call 911. For mental health crisis support, call or text 988.

8. Data sharing and third parties

We do not sell your mobile number. We may share information with service providers that help us deliver messages (for example, telecommunications or messaging platforms) as needed to operate the service, subject to confidentiality and applicable legal requirements. No mobile information will be shared with third parties/affiliates for marketing/promotional purposes.

9. Eligibility

You must be at least 18 years old, or have a parent/guardian enroll you, to opt in.

10. Changes to this policy

We may update this policy from time to time. The latest version will be posted on our website with an updated effective date.
